Yes. The breasts can be reshaped after implant removal, but not every patient needs additional surgery. Some breasts retract and settle well after removal alone. Others benefit from a breast lift or reduction when loose skin, low nipple position, or excess natural breast tissue would otherwise remain.
The best plan depends on what the breasts look like with the implants in place, how much of the current volume comes from the implants, and what shape you would like afterward. What Determines Breast Shape After Implant Removal?
Removing an implant reduces breast volume, particularly in the upper part of the breast. The amount of change varies considerably. Important factors include:
- The size of the implant relative to your natural breast tissue
- How long the implant has been present
- Skin elasticity and the amount of loose skin
- The position of the nipple and breast tissue
- Pregnancy, weight changes, and aging
- Previous breast operations and scars
- Whether capsular contracture or implant displacement is present
Early indentations and loss of upper-breast fullness can look pronounced immediately after surgery. These changes often soften as the tissues settle and heal, so the early appearance is not necessarily the final result.
When May Implant Removal Alone Be Enough?
Removal alone may produce a satisfactory shape when the nipples remain well positioned, the skin is not significantly loose, and there is enough natural breast tissue to create the desired contour. Some patients also prefer to avoid the additional scars of a lift, even if they understand that some looseness or loss of fullness may remain.
In less certain cases, a patient may choose removal first and allow the breasts to settle before deciding whether another procedure is worthwhile. This staged approach can show how much the tissues retract naturally, although it requires a second operation if a lift is later desired.
When Is a Breast Lift Helpful After Explant Surgery?
A breast lift, or mastopexy, may be helpful when the breast tissue has sagged, the nipple sits too low, or stretched skin would leave a shape the patient does not want after the implant is removed.
During a lift, Dr. Bogue removes excess skin, reshapes the patient’s own breast tissue, and repositions the nipple and areola. A lift does not replace the volume of the removed implant. Its purpose is to improve position and shape using the remaining breast tissue.
When Might a Breast Reduction Be the Better Choice?
Some patients still have relatively large or heavy breasts after their implants are removed. When excess natural breast tissue contributes to discomfort or an undesired size, a breast reduction can remove tissue while lifting and reshaping the breast.
The distinction matters: a lift primarily changes position and shape, while a reduction also decreases the amount of natural breast tissue.
Can Implant Removal and a Lift Be Performed Together?
Yes. Implant removal and breast lifting are often performed during the same operation when the need for reshaping is clear. Combining them allows Dr. Bogue to plan the breast around its new, smaller volume and avoids a second recovery.
Removal alone or a staged lift may still be appropriate when the likely post-removal shape is uncertain or when the patient wants the least extensive initial operation. The tradeoffs are individualized rather than determined by a single rule.
What Happens to the Implant Capsule?
Dr. Bogue performs a total capsulectomy with explant surgery, removing the scar capsule surrounding the implant and sending it to pathology. He does not use an en bloc cancer operation for routine explant surgery; that larger operation is reserved for known or suspected implant-associated cancer after appropriate evaluation.
How Does Reshaping Affect Recovery?
Recovery from implant removal alone is generally easier than recovery from the original augmentation. When a lift or reduction is added, recovery is influenced more by that additional procedure. Activity is increased gradually, and the breasts continue to soften and settle over the following months.
