How Does BOTOX® Cosmetic Work?
Botox works by temporarily blocking muscle movement, which causes them to relax. When injected into areas of dynamic lines (lines that form with movement), these areas look smoother and wrinkle-free.
BOTOX® Cosmetic primarily treats the following types of wrinkles:
- Glabellar lines (vertical lines between the eyebrows)
- Crow's feet (fine creases at the outer corners of the eyes)
- Horizontal lines across the forehead
- Bunny lines (small lines of the upper nose)
BOTOX® Cosmetic is different from injectable fillers like Juvederm®, which smooth wrinkles by filling them in. BOTOX® Cosmetic is a wrinkle relaxer which corrects creases formed not by volume loss, but by the repeated contractions of facial muscles.
When you visit Dr. Bogue for your injection appointment, the treatment area is iced to numb any potential discomfort. He then makes a series of small injections based on your desired results. Is Botox Safe For Everyone?
No. While Botox is safe for most patients looking to rejuvenate their appearance, not everyone is a suitable candidate. Patients who should avoid Botox injections are:
Pregnant or nursing.
Have allergies to ingredients in Botox.
Have underlying neurologic conditions, such as MS, ALS, or Ehler-Danlos syndrome.
Have open skin lesions in the treatment areas.
Who Is a Good Candidate for BOTOX Injections?
BOTOX® only works on wrinkles that form due to muscle contractions beneath the skin. These are known as dynamic wrinkles, and they form on the upper third of the face. BOTOX® works on crow’s feet at the outsides of your eyes, the 11s between your eyebrows, and on forehead lines. If you want to address wrinkles on the upper third of the face, BOTOX® is the answer. If your wrinkles are due to volume loss or sun damage, dermal fillers are the way to go.
How is BOTOX Different from Dermal Fillers?
BOTOX® works on what are known as “dynamic wrinkles.” These wrinkles form on the upper third of the face when we make certain expressions, such as showing surprise, frowning, squinting, and the like. If you make one of those expressions, you can feel the muscles around the eyes, on the brows, and on the forehead engage. Over time, as our skin thins, these contractions begin to form wrinkles on the surface skin above the muscle. These are dynamic wrinkles. When injected into the muscle forming the wrinkle BOTOX® erases the wrinkle by keeping the muscle from contracting.
Dermal fillers work entirely differently. They are injected beneath a wrinkle, crease, or area of volume loss and they simply “fill” in the space, pushing the depressed skin back upward. The most popular dermal fillers, Juvéderm® and Restylane, ® are made from hyaluronic acid, which occurs naturally in the skin and is responsible for plumping and hydrating the skin. Dermal fillers work on wrinkles caused by sun damage, declining collagen production, personal habits, and environmental factors. These are known as static wrinkles, and once the skin is damaged, they show themselves at all times.
Dermal fillers have no effect on dynamic wrinkles, and BOTOX® has no effect on static wrinkles.
What Is Botulinum Toxin Type A?
BOTOX® is basically made from the botulinum toxin type A along with some additional proteins. The bacteria in botulinum toxin are the same bacteria that cause botulism, which sounds scary, but it’s not. Back in the 1950s research scientists discovered that when injected in very minute amounts into a muscle the botulinum toxin caused that muscle to temporarily stop contracting. It does this by blocking the nerve messages sent to the brain. By blocking the message, the brain never receives the signal to contract the muscle, so the muscle stays at rest.
This led directly to the development of BOTOX®, which received its first FDA approval for treatment of involuntary eyelid spasms.
Are BOTOX Injections Painful?
No, Most patients tolerate the treatment well. However, topical numbing cream can be applied to ensure that you are comfortable during your injections.
What Can Botox Treat?
In addition to treating a variety of medical concerns, such as teeth grinding and neck spasms, Botox Cosmetic can be used to reduce the signs of aging around the eyes, between the brows, in forehead lines and wrinkles, in the chin and in the upper lip.
What Areas Can Be Treated with BOTOX?
People think BOTOX® can be used on any wrinkle, but as mentioned above, the wrinkles need to be a result of a muscle contraction under the skin. These wrinkles form on the expressive area of the face, the upper third. That is where the FDA approved cosmetic use of BOTOX®. When you make an expression, such as frowning or showing surprise, it engages muscles around your eyes and on your forehead. As we age, repeating these contractions hundreds of thousands of times begins to form wrinkles on the surface skin. BOTOX® blocks the nerve messages sent to the brain to contract these muscles, so the muscles stay at rest and the wrinkles never form.

When Will I See Results from BOTOX?
BOTOX® doesn’t provide immediate results. It takes about four days for BOTOX® to block the nerve messages and stop the muscle from contracting. From there, BOTOX® erases wrinkles for up to six months. At that point, the body will begin to absorb the now-inert botulinum toxin and the targeted muscles will begin to contract again, re-forming the wrinkles.
Can Botox Be Combined With Other Treatments?
Yes, Botox injections can be combined with other cosmetic treatments like dermal fillers. Many patients choose to have Botox and dermal fillers performed at the same time to create a more youthful, rejuvenated appearance.
Is There Any Recovery after BOTOX Injections?
There isn’t any recovery. That’s why BOTOX® was deemed to be the original “lunchtime treatment,” when it first came into the aesthetic world in 2002. Patients can come in for their BOTOX® session during a lunch break, have their BOTOX injections, and immediately go right back to work or their other normal activities.
The only thing to avoid after having BOTOX® injections is to not rub the injection sites for 24 hours. This lets the BOTOX® settle into place and keeps it from being pushed to an unintended muscle.
